Proxy Health Pass allows you to access and check in to Proxy enabled spaces.

1. To set up your Health Pass, once logged into the Proxy app, click the Start button in your Health Pass.

2. After reviewing the Health Pass related information in the next screen, click the Activate your Health Pass button.

3. In the next screen, select which document you wish to add to your Proxy app in order to activate Health Pass.

Vaccine certificate

- A vaccine certificate activates your Health Pass indefinitely. This is where you add photographs of your vaccination confirmation and certify its validity. The photographs are only stored locally on your phone.

Digital Vaccine Passport

The Proxy app is compatible with the following Covid-19 vaccine certificate QR codes:

• New York State Excelsior Pass
• California Digital COVID-19 Vaccine Record
• EU Digital COVID Certificate

If you have one of the above documents, you can activate Health Pass by scanning the QR code contained in your document.

Test result

- A test result activates your Health Pass for the duration of 14 days. This is where you add photographs of your Covid-19 test result. The photographs are only stored locally on your phone.

Health screen

- Proxy Health screen activates Health Pass until midnight. To complete the screen, you will need to answer a series of questions on any symptoms you may be experiencing, a potential exposure or a positive Covid-19 test.
