1. To submit a health screen and activate your Health Pass, click the Start button on the Add Health Pass card.
2. After reviewing the Health Pass related information in the next screen, click the Activate your Health Pass button.
3. Click Health screen tab in the next screen and confirm by click Start to begin the screening process. Please note that health screen results are valid only for 1 day and will expire at midnight every day.
4. Click the Start button to begin taking the questionnaire.
5. Follow the prompts and answer the questions relating to testing, symptoms and possible exposure situations.
6. Once done, your health screen will get added to your passes feed in the Proxy app. Depending on your answers you will either be assigned "Low Risk" or "High Risk".
